Per Klacid Hp (7 days) combi pack 14 Klacid tab each containing Clarithromycin 500 mg, 14 Controloc tab each containing Pantoprazole Na 40 mg, 14 Ospamox tab each containing Amoxicillin 1 g. Indications/Uses
Dosage/Direction for Use
Administration
Controloc: Should be taken on an empty stomach: Take 1 hr before meals. Swallow whole, do not chew/crush. Klacid & Ospamox: May be taken with or without food.
Contraindications
Klacid: Hypersensitivity to macrolides. Not to be given to patients w/ history of congenital or documented acquired QT prolongation or ventricular cardiac arrhythmia including Torsade de pointes; hypokalemia. Concomitant use w/ astemizole, cisapride, domperidone, pimozide, terfenadine, ergot alkaloids, oral midazolam, HMG-CoA reductase inhibitors, colchicine, ticagrelor, ranolazine. Not to be used in severe hepatic failure w/ renal impairment. Controloc: Hypersensitivity. Not to be used in combination treatment for H pylori eradication in patients w/ moderate to severe hepatic or renal dysfunction. Ospamox: Hypersensitivity to penicillins, cephalosporins. Not to be given to patients w/ mononucleosis & lymphatic leukemia; severe GI infections w/ persistent diarrhea or vomiting. Patients w/ allergic diatheses or bronchial asthma & hay fever.
Special Precautions
Discontinue use if severe acute hypersensitivity reactions or cutaneous ARs, including erythema multiforme, SJS, TEN, DRESS, acute generalized exanthematous pustulosis (AGEP) occurs. Pregnancy & lactation. Klacid: Superinfection may occur w/ long-term use. Patients w/ CV events, pneumonia, mild to moderate skin & soft tissue infections. Concomitant use w/ colchicine, HMG-CoA reductase inhibitors (statins), oral hypoglycemic agents/insulin & anticoagulants. Hepatic & moderate to severe renal impairment. Controloc: Exclude gastric malignancy prior to treatment. Discontinue use 5 days prior to chromogranin A measurements. Ospamox: Antibiotic-induced pseudomembranous colitis. Maintain adequate fluid intake & urine output during treatment. Concomitant use w/ antiperistaltic drugs.
Adverse Reactions
Headache, diarrhea. Klacid: Dyspepsia, vomiting, taste perversion; oral monilia. Controloc: Constipation, flatulence, peripheral edema, fever, depression, myalgia (upon discontinuation); erythema multiforme, subacute cutaneous erythematosus. Ospamox: Fatigue, fever, joint pain, angioneurotic edema, interstitial nephritis. Klacid & Controloc: Abdominal pain; allergic reactions, severe cutaneous ARs eg, AGEP, SJS, TEN, DRESS. Klacid & Ospamox: Nausea; stomatitis, glossitis.
Drug Interactions
Klacid: Potential severe drug interaction effects w/ cisapride, domperidone, pimozide, astemizole, terfenadine, ergot alkaloid, oral midazolam, HMG-CoA reductase inhibitors. Concomitant use w/ efavirenz, nevirapine, rifampicin, rifabutin, rifapentine, etravinine, fluconazole, ritonavir; antiarrhythmics, oral hypoglycemic agents/insulin, CYP3A-based interactions, omeprazole, sildenafil, tadalafil, vardenafil, theophylline, carbamazepine, tolterodine, triazolobenzodiazepines eg, alprazolam, midazolam, triazolam; colchicine, digoxin, zidovudine, phenytoin, valproate; atazanavir, Ca-channel blockers, itraconazole, saquinavir. Controloc: Reduced absorption w/ pH-dependent drugs eg, ketoconazole. Ospamox: May promote skin rashes w/ allopurinol. Not to be combined w/ bacteriostatic antibiotics eg, tetracyclines, chloramphenicol. Reduced tissue distribution & diffusion by probenecid. Reduced efficacy of OCs. Reduced absorption by antacid. False +ve non-enzymatic urinary glucose test result. Impaired urobilinogen tests.
